How To Start Your 4 Hour Mandatory Provider Training

  1. Click on the “Training” link on the top menu OR from the purple “Training” Box on the home page.
  2. You will be required to Register if you don’t have an account or Login if you do. If you have trouble logging in, please click the “lost password” link on the login page. This will reset your password if you have forgotten it.  If you still have an issue, please contact our webmaster at: [email protected]
    (Depending on which browser you are using, you will be offered a popup menu to choose your particular email provider to send your message. Just click on your email program and fill out your email)
  3. When you complete your Registration or Login, you will be taken to the Mandatory 4-Hour Training Course for FY 2021 page where you must be enrolled to take the course.
  4. Please read the instructions.  There are 3 modules that you must complete to gain your certificate.  They are all linked from that page.
  5. It is advised that you complete each module before moving on to the next module.
  6. There is one final quiz.  You must complete the quiz with a passing grade to receive your certificate. You will be allowed to take the quiz multiple times.
  7. When you are finished with each module please mark the module completed by clicking on the “Mark this completed” button.
  8. You will be issued a Certificate of completion link once you have finished your training after the final quiz.  Click on the link to download your certificate.  If you have any issues downloading your certificate, please contact our webmaster at: [email protected] (Depending on which browser you are using, you will be offered a popup menu to choose your particular email provider to send your message. How To Download Your Forms

Please note: We are currently updating the forms. It is currently offline for maintenance. We apologize for this momentary inconvenience.

  1. Click on the “Forms” link on the top menu OR from the orange “Forms” Box on the home page.
  2. You will be required to Register if you don’t have an account or Login if you do.
  3. When you complete your Registration or Login, you will be taken to the Forms and Downloads for: Center Ministry, FDC Homes and Infants page.
  4. Click on the link for the forms you wish to download.
  5. You can download the files you need individually or all together as a zip file
  6. When you are finished downloading the forms you need, please remember to logout.

How To Send Forms To Nutrition Inc.

  1. Click on the “Uploads” link on the top menu OR from the orange “Forms” Box on the home page.
  2. You will be required to Register if you don’t have an account or Login if you do.
  3. When you complete your Registration or Login, you will be taken to the Group Forms page. A folder will be created for you upon registration.
  4. Click on the link to upload a single file or on the “Select of Folder” link to upload a folder of forms to your created folder.
  5. You can also download Nutrition Inc  files from your created folder. These will appear in the file browser box on the Group Forms page. You can download these by clicking on the 3 vertical dots for the popup menu download link.
  6. When you are finished downloading/uploading the forms you need, please remember to logout.
